Community Care Services Program (CCSP)

Service Options Using Resources in Community Environments (SOURCE)

  • Elderly and Disabled Waiver
  • For adults ages 65 and older, unless disabled
  • Must have a chronic condition that has been present for at least 3 months and must meet nursing home level of care.
  • Must be eligible for SSI coverage not exceeding $781/ month

Structured Family Caregiving

The Structured Family Caregiving program (SFC) with FEHE offers financial and support services to Georgia families that deliver care at home for a senior or disabled person. You may meet SFC requirements and be eligible for financial compensation if:

    • You live with the person as the primary caregiver
    • Are related biologically as a daughter, grandson, etc. or are related through marriage as a daughter-in-law, son-in-law, etc.
    • The person you take care of is Georgia Medicaid-eligible and participates under the CCSP or SOURCE waiver program
    • Please note that spouses of waiver participants or legal guardians are not eligible for compensation.

Comprehensive Support Waiver (COMP)

  • Formerly Community Habilitation Support Services (CHSS)
  • Serves individuals’ with intellectual and developmental disabilities who choose to remain the community
  • Individual should be diagnosed before the age 22 and the diagnosis should be indefinite with an IQ <70
  • Individual should have 3 or more functional impairments:

     

      •  Self-Care
      •  Learning
      •  Mobility
      •  Self-Direction
      • Capacity for independent living
      • Requiring 24 hour care
  • Services Provided:
      • Adult PT/OT/ST
      • Behavioral Supports
      • Community Living Supports
      • Community Access
      • Prevocational Services
      • Supported Employment
      • Specialized Medical Supplies

  • Individual is diagnosed before age 22 with an IQ of <70

Community Access Group (CAG)

  • FEHE Day Program
  • For individuals with the NOW and COMP Waivers
  • Individual comes to the day center based on allotted budget

Community Residential Alternatives

Available to individuals who require intense levels of residential support in small group settings of four or fewer or in host home/life-sharing arrangements.

Services include a range of interventions that focus on training and support in one or more of the following areas: eating and drinking, toileting, personal grooming and health care, dressing, communication, interpersonal relationships, mobility, home management and use of leisure time
